The Centivo provider network is built around trusted, high-quality providers, including Prevea Health (including providers at Prevea Kohler Health Center), Ascension, Children’s Wisconsin, Froedtert, Independent Physicians Network, Medical College of Wisconsin and ProHealth Care as well as Lapcorp.
It is important to note Advocate Aurora Health’s network of doctors and hospitals are not included in the Centivo HPP network.
The Centivo provider network is also located only in the state of Wisconsin, but does offer virtual primary care options.
