The High Performance Plan is built around the relationship between you and your Primary Care Team. You’ll choose a primary care doctor at the start of your plan year (this is called “activation”). You can even select a provider at Prevea Kohler Health Center to lead your care. Your Team will get to know you and your healthcare needs, identify health issues early on and help you navigate the healthcare system.

As your partner and advocate, your Primary Care Team should be your first stop for all your care. If needed, they’ll refer you to trusted, in-network specialists and services. Under the High Performance Plan, visits with your Primary Care Team is always free. You’ll be charged simple, predictable copays for all other in-network care guided by your Primary Care Team.

The Centivo provider network is built around trusted, high-quality providers, including Prevea Health (including providers at Prevea Kohler Health Center), Ascension, Children’s Wisconsin, Froedtert, Independent Physicians Network, Medical College of Wisconsin and ProHealth Care. It is important to note Advocate Aurora Health Care is not an in-network provider on the HPP.
